Application of Membership
- You are welcome to join YMCA membership by submitting a completed membership application form, required documents, together with the corresponding membership fee, to our Member Services Counter in Tsim Sha Tsui YMCA Head Quarters. Or you may download and fill in the form, together with the corresponding cheque payment or credit card authorization form, and mail to:

Documents required for:
| Youth Membership | - Copy of ID, Passport or Birth Certificate - 1 recent colour photograph |
| Associate Individual Membership | - Copy of ID or Passport - 1 recent colour photograph |
| Family Membership | - Copy of ID or Passport of each family member - Copy of Birth Certificate of children under 17 years old - Copy of Marriage Certificate or Divorce Papers - Copy of official children adoption documents (if applicable) - 1 recent colour photograph of each family member (except for children under 6 years old) |
| Corporate Membership | - Copy of Business Registration Certificate - Softcopy of Company logo - 1 recent colour photograph of each corporate member (if applicable) |
Remarks:
- Cheque payment shall be payable to YMCA of Hong Kong
- Fees at date of acceptance will be applied and valid for one year
- Temporary membership cards will be issued once the application succeed
- Membership cards will mail out; or as request to pick up at our Member Services Counter by returning the temporary membership cards, if no temporary membership cards present when collection, proof of identification is needed
Personal Information Collection Statement
The information provided by you will be used for purposes relating to the administration in the Member Services Section.
This Section may give some of the information to other parties within the Association relating to your usage of our facilities and courses taken.
Subjectto exemption under the Personal Data (Privacy) Ordinance, you have the right of access and correction with respect to personal data.
Request for personal data access and correction should be addressed to Members Services Manager, and please quote your membership number.
